Primary Care Physician PACE Program

Position Summary :

Under the direction of the Chief Medical Officer, the Primary Care Physician (PCP) is responsible for the medical care of participants enrolled in the PACE program . The PCP provides medical care, leadership for the PACE team, and guidance / education to interdisciplinary team (IDT) members regarding medical issues. This position works collaboratively with clinic leadership and site managers to ensure effective medical care and models a collaborative partnership with families and caregivers to address participants medical and non-medical needs.

Salary Range : $198,000 $316,000 per year, based on market conditions, experience, skills, abilities, and work history.

Duties and Responsibilities

Direct Patient Care

  • Provide comprehensive coordinated medical care as guided by the PACE benefit
  • Conduct comprehensive initial, semi-annual, annual, and periodic assessments of participants
  • Participate with the IDT in developing and implementing care plans; review and sign plans every six months
  • Update care plans and inform the IDT as new issues arise
  • Provide primary care assessments and treatment, order diagnostic tests / medications, and arrange follow-up care
  • Oversee appropriate specialist referrals and coordinate care with participants and their families
  • Provide on-call telephonic medical services for after-hours coverage on a rotating basis

Collaboration & Leadership

  • Supervise nurse practitioners and physician assistants as appropriate
  • Coordinate care for hospitalized participants and guide discharge planning
  • Manage care for nursing home participants as assigned, ensuring care plan adherence
  • Maintain a close, collegial working relationship with IDT members
  • Provide medical guidance at provider / clinic meetings, care conferences, and family meetings
  • Participate in committees, task forces, and quality improvement initiatives as needed
  • Administrative & Documentation

  • Document all clinical interactions in the electronic medical record, ensuring records are accurate and up-to-date
  • Assist clinic and administrative staff to ensure efficient operations
  • Participate in quality assurance performance improvement (QAPI) activities
  • Maintain confidentiality and comply with HIPAA, organizational policies, and safety protocols
  • Qualifications

  • MD degree and current, unrestricted license to practice medicine in California
  • Board certification in Internal Medicine or Family Practice (Geriatrics preferred)
  • Minimum of 1 year of experience working with a frail or elderly population (PACE experience preferred)
  • Current DEA certification
  • Current Basic Life Support (BLS) certification
  • Strong communication, interpersonal, and leadership skills
  • Ability to work effectively in a multicultural, interdisciplinary team environment
