Job Summary And Qualifications

The Director of Patient Navigation will drive operational excellence for HCA's navigation program, specifically ensuring delivery of high quality, evidence-based care through program development under the direction of the Care Navigation leadership team and in collaboration and alignment with Corporate and Division Service Line leadership teams. The Director will serve as a regional leader for executing mid and long-range strategic initiatives to optimize operational performance and coordinate various operational tactical plans while tracking performance against key performance indicators. He / she will provide expertise, initiative, leadership and programmatic oversight to division and facility leadership across navigation programs in their respective region to elevate navigation program performance and ensure high-quality healthcare services for HCA patients. The Director will provide oversight and leadership to the Manager and Supervisors (as applicable) of Care Navigation and further support for both local and remote nurse navigators.

Patient navigation programs function to support the navigation of patients through complex healthcare environments for the optimization of care and outcomes, while maximizing patient retention. Navigators facilitate patient follow-up through coaching, information transfer, and communication in order to improve continuity of care, enhance the medical management of complex disease, and streamline the intervention process. Navigators guide patients through the health care system by identifying barriers to care, linking to needed resources, and ensuring adherence to medical treatment plan.

In this role you will :

  • Collaborate with division and facility leadership teams and Corporate leadership to implement program improvements that are consistent across the enterprise
  • Execute on strategic initiatives to maximize navigation to support service line goals
  • Collaborate with service line leaders to ensure connectivity, intelligence sharing, and alignment
  • Motivate and lead a high-performance team of navigation managers across service lines; attract and retain talented team members, provide mentoring and develop top talent potential
  • Develop a thorough understanding of the different service line navigation programs
  • Evaluate quality and effectiveness of division navigation programs, analyzing appropriate internal and external data to identify opportunities to improve service and outcomes
  • Educate key stakeholders on the role and benefits of navigation programs through ongoing outreach
  • Identify and implement strategies to improve patient retention through navigation tactics
  • Monitor and report out on program performance, including programmatic, outcome, and financial metrics with a focus on efficient, actionable communications
  • Implement and provide ongoing support for group network navigation model that will support all navigation services across the enterprise
  • Develop and leverage excellent working relationships with Corporate Executives and Service Line Leaders, Division leadership, Division Service Line Leaders, Facility CEOs, COOs and VPs, relevant physicians, Marketing Directors and other key stakeholders as appropriate
  • Identify, develop and maintain relationships with strategic physician partners, in collaboration with the division's Physician and Provider Relations team
  • Collaborate with Division, Corporate and Service Line leadership in evaluating the FTEs needed to support Service Line navigation, including expansion and realignment of various navigation programs.

Qualifications that you will need :

  • Bachelor's degree - Required
  • 5+ years of experience in a leadership role - Required
  • 3+ years of experience in program development - Preferred
  • 1+ year(s) of experience in service line leadership Preferred
  • Ability to travel up to 50%
