About the Role

The Direct Care Professional plays a vital role in providing compassionate and personalized support to individuals in need of assistance with daily living activities. This position focuses on enhancing the quality of life for clients by promoting independence, dignity, and respect in a safe and supportive environment. The role requires close collaboration with healthcare providers, family members, and other team members to ensure comprehensive care plans are effectively implemented. The Direct Care Professional will be responsible for observing and reporting changes in clients' conditions to facilitate timely interventions. Job Responsibilities

• Assist clients with activities of daily living such as bathing, dressing, grooming, and meal preparation.
• Provide emotional support and companionship to promote social engagement and mental well-being.
• Monitor clients' health status and report any changes or concerns to the supervisory healthcare team.
• Implement individualized care plans and support clients in achieving their personal goals.
• Maintain accurate documentation of care provided and communicate effectively with team members and family.
