Were hiring for a Physical Therapist role on our Mobile Team which delivers outpatient physical therapy directly in patients homes. This role also includes the use of telehealth services to support patient care.

The ideal candidate is :

• Values flexible scheduling which may include daytime or evening hours based on clinician and patient preferences

Primary coverage area : DeKalb / Sycamore / Sandwich area
