CONCERN is currently seeking a skilled and compassionate Certified Registered Nurse Practitioner (CRNP), Nurse Practitioner (NP), or Physician Assistant (PA) to join our team on a contract basis to support our Outpatient Mental Health Offices and Shelter Program in the Berks County area. This independent contractor position offers the flexibility of both in-person care and telehealth delivery, providing vital services to Children, Adolescents, and Adults with behavioral health needs in a community-focused setting.

Position Highlights :

• Provide psychiatric evaluations, medication management, and ongoing psychiatric care for individuals in our outpatient and shelter-based programs.
• Collaborate with interdisciplinary teams to support continuity of care and ensure client success.
• Maintain accurate and timely documentation in accordance with applicable regulations and standards.
• Offer services via telehealth when appropriate to enhance accessibility and continuity of care.
