Now Hiring :    Nurse Practitioners (Lancaster / York,  PA )

Hours :  Per Diem ( Flexible schedule)

The  Nurse Practitioner  will be responsible for, but are not limited to :

• Conduct visits to patients on palliative care in accordance with local, state and federal regulations.
• Communicate visit findings to designated members of the interdisciplinary team, including the National Director of Palliative Care.
• Facilitate access to appropriate support models of care (i.e., hospice, rehabilitation, on-going aggressive treatment).
• Consult with other members of interdisciplinary team to contribute to the patient's plan of care.
• Prepare and maintain accurate patient records, charts and documents.
• Assist in development of clinical practice guidelines / standards in support of quality palliative care as needed.
• Ensure collaboration with patients, caregivers, medical directors and members of IDT.

Education :

Master's Degree in Nursing
