Job Title : Physical Therapist

Job Specialty : Home Hospice Care

Job Duration : 13 weeks

Shift : 8 Hour Days, Monday to Friday

Guaranteed Hours : 40 hours per week

Experience : Willing to accept new graduates and first-time travelers

License : South Dakota Physical Therapy Licensure

Certifications : BLS, National Accreditation through NPTE required

Must-Have : Doctorate in Physical Therapy

Job Description :

• Provide physical therapy services to medically compromised patients, including those with CHF, COPD, DM, and cardiac conditions, as well as ortho, neuro, dementia, and general weakness.
• Evaluate patients and determine effective intervention and care plans.
• Conduct medical assessments and develop tailored care strategies for patients ranging from early adult to geriatrics.
• Work collaboratively with a Home Health Department team including RNs, LPNs, OTs, PTAs, SLPs, Aides, and MSWs, and case managers.
• Utilize EPIC for charting and documentation.
• No specific equipment skills required.
• Position involves floating within the Southern Black Hills area, including specific locations such as Hot Springs, Custer, Edgemont, Hill City, Hermosa, and surrounding areas.
• Complete orientation, approximately one week, including EPIC training, shadowing, and mastering documentation processes.
• No weekend rotation or on-call duties; office closed on major holidays.
• Scrub color of choice or business casual attire allowed; no denim or sweats permitted.
