Job description

We're Hiring : Field Nurse Assessor!

We are  hiring  Registered Nurses (RNs) for the  Independent Assessor (IA) Project , conducting  in-home and telehealth assessments  for individuals applying for community-based long-term  geriatric care  programs.

Location :   Brooklyn, NY

Role :   Field Nurse Assessor

What You'll Do

  • Conduct UAS-NY assessments in-home or via telehealth.
  • Review medical records and ensure accurate documentation.
  • Educate consumers and coordinate care with providers.
  • Manage your own appointments and maintain compliance.

    • What You Need

  • 1–2 years  of experience as an RN in one-on-one in-home, community, skilled nursing facility, long term and geriatric care required
  • RN license valid in  New York
  • BSN  is required
  • Familiarity with  Uniform Assessment System (UAS)-NY  is a plus
  • Strong communication, time management, and Electronic Health Record (EHR) skills
  • Reliable transportation for in-person visits
