Will consider locum, locum to perm, and perm providers.

Practice Setting : Neurology clinic

Reason for Coverage : Vacancy

Dates of Coverage : As soon as credentialed

Rate Type : Hourly

Schedule : Monday Friday, 8 : 00 AM 4 : 30 PM

Practice Demographics :

• 95% Adult
• 5% Pediatric

Patient Load :

office / clinic patients per day (7.2 hours per day)

Other Neurologists Present : Yes

0 Advanced Practice Providers

Office Ancillary Staff :

1 RN Navigator

4 Medical Assistants (MAs)

3 Customer Service Representatives (CSRs)

Documentation System / EMR : Epic

Trauma Level : 2

Board Requirements : Board Certified / Board Eligible

State License Preference : Prefer providers with an existing license to expedite the process

Certification Requirement : BLS

Required Neurology Procedures :

Radiologic Exam Interpretation (Unofficial)

EMG (Electromyogram)

Botulinum Toxin Administration (Botox)

Estimated Credentialing Timeframe : 120 days
