We are seeking a dedicated Physician (MD / DO) to provide comprehensive, coordinated primary care for participants in a PACE (Program of All-Inclusive Care for the Elderly) setting. Schedule : Monday–Friday, 8 : 00am–5 : 30pm
Key Responsibilities
- Perform comprehensive initial, 6-month, annual, and periodic assessments.
- Develop, review, and update individualized care plans in collaboration with the interdisciplinary team (IDT).
- Provide primary care, ongoing medical management, and coordinate diagnostic tests, medications, and specialty referrals.
- Participate in IDT meetings, care conferences, and family meetings.
- Provide after-hours telephonic on-call coverage as part of rotation.
- Coordinate care for hospitalized and nursing home participants; support smooth transitions and follow-up.
- Document all clinical interactions in the EMR ( eClinicalWorks
- Lead and participate in provider meetings, task forces, and quality improvement initiatives (QAPI).
- Maintain accurate medical records and ensure compliance with all policies, safety protocols, HIPAA requirements, and training standards.
