Oklahoma Licensed Care Manager

A company is looking for a Care Manager (RN) to develop and facilitate complex care management activities for members with physical needs.

Key Responsibilities

Evaluate member needs and barriers to care, recommending plans for optimal outcomes

Develop ongoing care plans and collaborate with providers to address unmet needs

Coordinate between members, families, and care providers to ensure person-centered care

Required Qualifications

Degree from an Accredited School of Nursing or Bachelor's degree in Nursing

2 - 4 years of related experience

RN - Registered Nurse - OK State Licensure and / or Compact State Licensure required
