Job descriptionWork closely with HCBS Supervisor and HCBS Programmer to create and implement goal-oriented plans for individuals. Deliver personalized support services based on each individual's unique goals and behavioral care plan, which may include teaching new skills, employing behavioral strategies, encouraging participation, and maintaining a safe environment. Accurately document an individual's progress throughout the duration of the service, highlighting successes and areas for improvement. Assist individuals in creating a safe and supportive living environment by instructing them on best practices for reaching their goals. Act as an advocate for individuals, promoting their autonomy and enabling them to make informed decisions about their lives. Opportunity to make a meaningful difference in the lives of others.
