Join our team to be the ultimate guide for clients navigating our Pathways, Bridge, and CE&T programs! You'll be the friendly first point of contact, juggling referral calls, explaining service options, ensuring a seamless journey from initial inquiry to start date and beyond. This position will also be responsible for following through with PCP and ISP meetings and plans for existing participants, as well as other duties as the role evolves.
What You'll Do :
- Master our services, funding sources, and program details to connect clients with the perfect care plan.
- Keep our referral tracking sheet fresh and accurate in real-time!
- Build strong relationships with care managers and providers to boost referrals and support clients beyond our scope.
- Coordinate smooth transitions for CE&T SE clients to Pathways LTVS, guiding them from EIPD closure to authorized services.
- Walk clients through tailored care transitions, syncing with care managers, families, and staff like a pro.
- Set up PCP and ISP meetings for new and existing participants, draft plans, handle documentation, and ensure a stellar program kickoff.
- Run the admissions committee, prepping referrals and keeping our admissions spreadsheet on point weekly.
- Support programs with Quality Improvement objectives and chair the Quality Improvement Committee.
- This is a newly created position, and additional duties that align with the role's objectives may be assigned as the position evolves.
What You Bring :
