About The Role
Join a rapidly growing, value-based healthcare organization that’s transforming how chronic care is delivered. This role offers the opportunity to provide comprehensive, in-home care to patients living with complex chronic conditions such as kidney disease and related metabolic disorders.
As an Advanced Practice Provider (NP or PA) , you’ll play a critical role in improving patient outcomes through hands-on clinical care, disease education, medication management, and care coordination—delivered directly in the patient’s home.
What You’ll Do
- Conduct initial assessments, follow-ups, and hospital transition visits.
- Perform vitals, head-to-toe physical exams, and hands-on patient assessments.
- Develop and manage individualized care plans and medication regimens.
- Provide chronic disease education to empower patients in self-management.
- Visit 4–5 patients per day on average.
- New patient visits : ~90 minutes
- Follow-ups : 30–60 minutes
- Participate in team rounds to discuss high-risk patients and coordinate updates.
- Document all encounters in Athena EMR, ensuring seamless communication with the broader care management team.
Schedule & Travel
Hybrid model : 3–4 days of in-home visits, 1–2 days remote (admin, telehealth, charting).No on-call or weekends.AI-optimized scheduling to streamline daily routes and patient assignments.Compensation & Benefits

Master’s-prepared Nurse Practitioner (NP) or Physician Assistant (PA).Minimum 2 years of APP experience.Background in home health, primary care, or internal medicine preferred.Experience in managing chronic care plans and collaborating within interdisciplinary teams is highly valued.Strong communication skills and a patient-first approach to care.Why Join Us
