Description :

We are seeking experienced RN Care Managers for a 13-week assignment.

These positions offer 40 hours per week with variable shifts of either 8 : 30 am to 4 : 30 pm or four 10-hour shifts from 7 : 30 am to 5 : 00 pm.

There is no on-call duty required, but weekend and holiday rotation is mandatory.

The RN Care Manager will coordinate concurrent utilization management activities house-wide.

They will serve as the main link between the patient, healthcare team, organization, and payors during the patient's hospital stay.

Key responsibilities include conducting assessments of patient needs and identifying resources required for transitions to the next setting.

They will collaborate, educate, and lead an interdisciplinary team during daily care rounds, care conferences, and care planning.

Excellent communication skills are essential for written, verbal, and electronic interactions.

The RN Care Manager must be able to work with community organizations to secure durable medical equipment and arrange transportation.

Participation in managing the length of stay for an inpatient hospital population is required.

Knowledge of hospital reimbursement methods and applicable quality measures is essential for the role.

An Associate or Bachelor degree in nursing is preferred, along with relevant clinical experience in medical / surgical nursing, home care, or hospital case management.

Certification by a nationally recognized case management or utilization review organization is preferred.
