Job Summary
As a Care Management Specialist, you will play a critical role in coordinating integrated member care across the healthcare continuum. You will collaborate with a multidisciplinary team to help members achieve their health goals while ensuring quality and cost-effective care.
Essential Job Duties
- Conduct assessments of members within regulated timelines to identify those who may benefit from care management.
- Develop and implement personalized care plans alongside members, caregivers, healthcare professionals, and support networks.
- Engage in telephonic, face-to-face, or home visits as necessary.
- Monitor care plans regularly to assess their effectiveness, document interventions, and adjust goals based on progress.
- Maintain a caseload, providing ongoing outreach and management to members.
- Promote the integration of services including behavioral health and community resources to enhance continuity of care.
- Lead interdisciplinary care team meetings and facilitate informal collaboration among team members.
- Utilize motivational interviewing techniques and clinical guideposts to engage and support members.
- Identify and address barriers to care through effective coordination and assistance.
- Collaborate with licensed care managers and leadership as needed.
- Be prepared for estimated local travel of 25-40%, depending on contractual obligations.
Required Qualifications
Minimum of 2 years of experience in healthcare, particularly in care management or medical / behavioral health settings.Clinical licensure / certification required based on state regulations.Valid driver’s license, reliable transportation, and appropriate auto insurance for work-related travel.Sound knowledge of community resources and the ability to work proactively.Adaptability to diverse populations and situations.Strong communication skills, both verbal and written, with a calm approach in high-pressure scenarios.Excellent time management, prioritization, and problem-solving skills.Proficiency in Microsoft Office and relevant software.A degree in a healthcare-related field may be necessary based on state requirements.Preferred Qualifications
