Essential Duties and Responsibilities :
The Adult Skills Center (TASC) provides innovative and proactive services that empower individuals with diverse intellectual, developmental, and mental health needs to achieve their highest level of Independence and realize their greatest potential. The Clinical Case Manager acts as the liaison between TASC, the client, the IPP team, and generic resources. They provide support to the client and direct care staff, spearhead the assessment and case management and track documentation of services and all interactions.
Primary Duties :
Client EngagementWork as part of a multi-disciplinary team to assess client needs. Design, modify and implement individualized treatment plans
Design and modify individualized treatment plans with the support of the program managerDocument changes in client's conditions and take necessary actionFacilitate trainings, health education and medication managementParticipate in Circle of Support, IPP, ISP and other meetingsCoordinate with medical providers and pharmacy for ongoing care. Assist with making and transporting clients to medical and other appointments as needed.Educate and support clients with behaviors to adhere to their overall care planServe as a liaison between TASC, client, the IPP team, and generic resources following best practices and point of contact standards. Assists client with maintaining open communication.Identifies and utilizes community resources to meet client needsPrepare reportsAttend planning meetingsTraining and collaboration with staff and other service providers. TEAMWORK with professionals from other disciplines is essentialTrain, supervise, and evaluate behavior therapistsWork well under pressure, ability to prioritize, and has strong time management skillsFoster team engagement and growthProgram ManagementSchedule client coverage and identify emergency coverage as needed
Efficiently utilize the time record keeping system, including claiming hours for direct care staff and ensuring payroll information is accuratePerform weekly in person visits, direct observation and support of clients and teamsPrepare and submit SIR’s and appropriate documentation to funding agencies, APS, and licensing within the specified time period with support of a program managerDocument provided services for monthly billingReview and analyze daily notes and dataAssist in crisis management as neededCoordinate and maintain both client based and administrative responsibilitiesDraft and present disciplinary actions to employees per TASC proceduresProvide support as needed to other departments per Director instructionTASC LeadershipRepresent TASC within the community
Participate on community boards and / or committees in order to enhance community knowledge of TASC as well as provide TASC with relationships with additional community resourcesFoster team engagement and growthOther duties as assignedQualifications :
Active RN / LVN / LPT licensure in good standingAt least 3 years of experience providing treatment to adults with developmental disabilitiesStrong computer skills, including demonstrated success with Microsoft Office Suite; strong skills and comfort with internet applications and social networking sitesMust be able to successfully complete First Aid / CPRMaintain all certifications up to dateMust have current California Driver’s license / Insurance and clean DMV.Must receive criminal record clearance through the DOJ.Desire to compassionately align with the client, while supporting a culture of creating self-reliance and sustainabilityAbility to establish and develop effective relationships with staff, employers, volunteers, community groups, current and potential donors, corporate sponsors, agency partners and other key stakeholdersMust be able to successfully complete PCMA training within the first 90 days of hire.Physical Requirements :
Ability to drive for workAbility to safely and successfully perform the essential job functions consistent with the ADA, FMLA and other federal, state and local standards, including meeting qualitative and / or quantitative productivity standards.Ability to maintain regular, punctual attendance consistent with the ADA, FMLA and other federal, state and local standardsWhile performing the duties of this job, the employee is frequently required to stand, walk, and sit. The employee must occasionally lift and / or move up to 50 pounds.The employee must provide physical assistance and / or push clients up to 250lbs in a wheelchair. Must be able to assist adult clients transitioning in and out of vehicle and in the restroom.
Completion of Professional Crisis Management Certification within 90 days of hire (provided by TASC) is required for this position and is not recommended for :
1) Individuals who have mobility and / or range of motion problems
2) Individuals with recent fractures or surgeries
3) Individuals who have suffered any kind of joint injury requiring medical attention in the past 6 months
4) Individuals with poor balance, flexibility or agility
