Key Responsibilities
Provide skilled nursing care to home health patients, assessing physical, psychosocial, and environmental needs
Develop and manage individualized plans of care in collaboration with patients, families, and interdisciplinary teams
Conduct initial and follow-up evaluations to monitor patient progress and adjust care plans accordingly
Communicate significant findings, changes in condition, or safety concerns to supervisors and physicians
Maintain accurate, timely clinical documentation and uphold HIPAA privacy standards
Coordinate care and ensure continuity between all parties involved in the patient’s treatment plan
Qualifications
Current, unrestricted Registered Nurse (RN) license
Minimum 1 year of recent clinical nursing experience
Valid driver’s license, reliable transportation, and active auto insurance
Willingness to travel within assigned service area (up to 50%)
Associate or Bachelor’s degree in Nursing (BSN preferred)
Prior Home Health or Hospice experience a plus
Familiarity with electronic charting systems such as HomeCare HomeBase preferred
