LPN Case Manager Assistant

A company is looking for a Case Manager Assistant (Disease Management) / LPN.

Key Responsibilities

Assist the RN Disease Manager with Disease Management functions and maintain communication with healthcare team members

Review medical data and coordinate care arrangements based on patient needs and available benefits

Document case actions in the computer system and prepare reports for payers under the direction of the Disease Manager

Required Qualifications

Licensed Practical Nursing Education required

5 years of varied clinical experience preferred, or 3 years in a specialty field

Basic computer skills

Good organizational and time management skills

Familiarity with community resources and funding sources
