Job Summary :
Finger Lakes Community Health is seeking a dedicated Behavioral Health Clinician to join our team at the new School-Based Health Center located within the Penn Yan Central School District. This is a unique opportunity to provide meaningful, year-round support to eligible students from kindergarten through 12th grade.
The clinician will deliver high-quality psychotherapy, clinical assessment, and evaluation services to address behavioral, developmental, emotional, and social challenges. All care will be provided in accordance with the New York State Department of Health Article 28 regulations for Diagnostic and Treatment Centers.
This position follows the district’s academic calendar, including summer school hours, based on the board-approved school schedule. This includes regular school days, scheduled breaks, paid holidays, and summer school hours. Any additional days that the school is closed outside of the anticipated schedule will be subject to FLCH policy and may still require work to be completed at another FLCH facility.
Schedule :
During School Year : Monday–Friday, 8 : 00 AM–4 : 00 PM (follows the school calendar)
During Summer School : Monday–Friday, 8 : 00 AM–1 : 00 PM
Additional Hours : Occasional evening or weekend hours may be required to attend school-related events such as parent-teacher conferences, school board meetings, or community engagement activities.
Additional Offererings : Hours may also be available at one of our health centers when the school is closed as an option to make up additional work time.

Main Responsibilities :
Formulate and complete assessment
Provide counseling and psychosocial plans to address the following :
mental, social, emotional, behavioral disorders and problems
developmental and addictive disorders, conditions and disabilities
Use assessment instruments to identify and evaluate disorders and dysfunctions of mental illness.
Utilize various therapeutic inventions
Provide mental health counseling using evidence based approaches (this will be up to provider and patient needs). This may include but not limited to :
Cognitive Behavioral Therapy
Dialectical Behavioral Therapy
Solution Focused Therapy
Motivational Interviewing
Develop and implement treatment plan
Work collaboratively with individual on individualized treatment plan that includes the following :
Diagnosis / assessment
Goal
Objective
Barriers to treatment
Strength of individual
Modality and Intervention used
Target date
Coordinate care with an integrated focus
Team based coordination with primary care
Participate in behavioral health case conference
Be a core member with a multidisciplinary care team
Participate in a planned supervision for license and consultation
Documentation
Progress notes and treatment plans are documented in an integrated electronic medical record. Notes will include the following but not limited to :
Presenting problem and status of symptoms
Treatment goals
Progress towards goals
Functional status
Education and Qualifications :
A current unrestricted NYS Licensed is required
CPR certification required (can be provided)
Strong interpersonal skills and the ability to build relationships with personnel, patients, and community partners
Proven ability to handle confidential information with discretion
The ability to work flexible hours some evenings are required
The ability to communicate fluently in both Spanish and English is preferred
Physical Requirements :
Must be able to hear and communicate with clients and staff via phone, web media, and in person.
Must be able to lift up to 50 lbs
