HWC - RN Case Manager

Hospice of Washington County

Location : Serving Hagerstown, MD and West Virginia.

The RN Case Manager will support Hospice of Washington County and will report to the Team Manager. He / she is responsible for identifying patient / family / facility care needs, and for providing nursing and supportive care for the terminally ill patient and their families.

Education :

Diploma, ADN and / or BSN from an accredited institution

Licensures / Certifications :

• Current Maryland State license as a Registered Nurse

Experience :

Two years of nursing experience, or equivalent combination or education and experience.

Pay Range

$64,992.56 - $99,876.12

Final salary offer will be based on the candidate's qualifications, education, experience and alignment with our organizational needs.
