Expected Starting Salary Range: $21/hr
This community-based position requires meeting clients in their homes, community, schools, etc. This requires the ability for the potential candidate to transport clients in their own vehicle to/from appointments, school, community events, etc.

Working as a BHS Community Case Manager providing services designed to provide specific, measurable, and individualized services to each client served, you will:
·Assist in achieving personal independence in managing basic needs as identified by the individual and/or parent or guardian by providing an ongoing assessment of needs
·Facility further development of daily living skills, if identified by the individual and/or parent or guardian, working with natural support systems in the community
·Coordinate and/or assist in crisis management, stabilization, advocacy, and outreach as needed
·Deliver services in the school setting and are trained to work with various challenges facing youth, such as anxiety, depression, ADHD, major life changes and difficulty with social relationships. Sessions are held during the school day, which provides a great deal of convenience for busy families.
·Document accurately and timely the therapeutic interventions provided and response of patients/families/significant adults in the electronic medical record (Epic) per state requirements and established quality standards
Qualifications
Job Requirements:
- Bachelor's Degree in a related field OR
- High school diploma or equivalent AND 2 years of work experience in a related job discipline
- Must maintain less than six points on valid driver’s license
Preferred Qualifications:
- Community mental health experience
- Case management experience
