LPN Full Time Position - 5x8 Schedule

Position Overview

Full-time Licensed Practical Nurse opportunity with 5x8 shifts

Day, Evening, Night, and Weekend rotations available

Holiday work required

Focus on providing age-appropriate care for adult and geriatric patient populations

Essential Qualifications

Current New York LPN Licensure required

Basic Life Support (BLS) certification required

Minimum 1 year of experience serving in an LPN capacity

Minimum 1 year of experience in Long Term Care (LTC) facility preferred

Position Requirements

Demonstrate knowledge and skills necessary to provide care appropriate to the age of patients served

Show understanding of principles of growth and development across the life span

Assess data reflective of patient status and interpret appropriate information

Identify each patient's requirements relative to age-specific needs

Provide direct and indirect skilled nursing care for assigned residents

Follow defined policies and procedures under supervision of a Registered Nurse

Maintain collaborative relationships with nursing staff, physicians, and ancillary staff

Maintain appropriate level of knowledge and clinical expertise through continuing education and in-service programs

Provide specialized care to geriatric residents

Key Responsibilities

Collect data relevant to resident's physical and psychosocial condition on ongoing basis

Communicate findings to RN and other members of the Interdisciplinary Team

Contribute to creation of developmentally age appropriate and culturally relevant plan of resident care

Reflect nursing diagnosis and interventions to achieve desired resident outcomes

Involve resident / family and healthcare team in care planning process

Promote healthy, safe environment for residents

Recommend revisions to plan of care after evaluation of resident outcomes

Indicate need for referrals to ensure all aspects of resident care are addressed

Collaborate with resident / family and Interdisciplinary Team members within scope of nursing practice

Assure customer satisfaction throughout care delivery

Maintain current knowledge and competency in Nursing Practice

Participate in mentoring / orientation of staff

Consider factors related to safety, cost, and impact in planning and delivery of care

Provide leadership in professional practice setting under direction of RN

Demonstrate knowledge and competency in all assigned duties

Work Environment Considerations

Occupational exposure to blood borne pathogens is reasonably anticipated

Required tasks involve potential contact with blood, body fluids, or tissues
