Description :

Catholic Services of Long Island have Physical Therapist Home Health needs.

There are several areas requiring services, and preferences regarding teams should be confirmed prior to submission.

Key task includes reviewing patient charts and medical history.

Developing personalized treatment plans is essential.

Therapeutic exercises and stretches will be taught to improve patients' conditions.

Hands-on therapy will be used to promote healing.

Maintain patient records by tracking goals and progress while adapting treatment plans accordingly.

This role requires close collaboration with a supportive patient care team including physicians, nursing, social services, and home health aides for your assigned caseload.

The patient age groups served include adults and geriatrics.

Responsibilities include seeing 5-7 cases per day and rotating weekends.

Mileage will be reimbursed at the IRS rate.

This position emphasizes the need for travel compliance and ensures that all certifications are included during the hiring process.

All applicants must confirm BLS certification is AHA compliant to qualify.

Candidates with prior travel assignments will be preferred, but those with substantial experience will also be considered.
