We are seeking a compassionate and dedicated Social Work Registered Intern to join our integrated care team. This is a unique opportunity to work closely with diverse patients and families, applying the principles of social work to improve lives through assessment, intervention, advocacy, and support services.
In this role, you will play a vital part in addressing behavioral health, substance abuse, and social support needs in both individual and group settings. Working within a multidisciplinary team, you’ll help patients navigate complex health and life challenges while ensuring culturally competent and developmentally appropriate care.
Key Responsibilities Include :
Conduct clinical assessments, develop treatment plans, and deliver individual, family, or group therapy.
Provide case management, referrals, advocacy, and support services across a wide range of patient needs.
Deliver substance abuse counseling, relapse prevention strategies, and patient / family education.
Maintain accurate documentation of all services, treatment plans, and progress notes in alignment with HIPAA standards.
Coordinate care with internal and external providers, and support patients through community resource navigation.
Participate in outreach, workshops, and professional development activities.
Uphold the highest standards of ethics and social work practice, including mandatory reporting and trauma-informed care.
Minimum Qualifications :
Master’s degree in Social Work (MSW) from an accredited program.
Registered Intern status with the state licensing board.
At least 1 year of experience in substance abuse counseling.
Proficiency with Microsoft Office Suite and general computer skills.
Reliable transportation.
Preferred Qualifications :
Experience using Electronic Health Records (EHR).
Previous experience in a community health or behavioral health setting.
