Team Leader Assertive Community Treatment

The ACT / ACT TAY Team Leader establishes, administers and directs the program of Assertive Community Treatment (Transitional-Aged Youth) ensuring appropriate services are provided by a multidisciplinary team and functions as a practicing clinician on the team. Understanding of Co-occurring program that works with women that have young children, preferred.

This is a full-time, benefit eligible position working primarily Monday-Friday.

Responsibilities Include :

• Provide supervision of ACT team members to include at least one hour of face-to-face individual supervision, day, weekend, and evening availability for consultation and ongoing opportunity for skill development
• Ensure appropriate documentation and recording occurs within individual case records per policy
• Participate in daily team meetings
• Ensure face-to-face initial screenings occur upon referral to the ACT program
• Conduct screenings and assessments for potential consumers
• Provide regular reports to program director on budget and daily operations of the program
• Ensure the multidisciplinary team remains fully staffed
• Other duties as assigned

Operating vehicles or machinery

Master's degree in social work or related field required

At least six months experience working with persons with serious and persistent mental illness required

LICENSURE / CERTIFICATION : PLPC, LPC, LMSW or LCSW valid in the state of Missouri required.
