Minnesota Licensed Clinical Pharmacist

A company is looking for a Clinical Pharmacist - Remote.

Key Responsibilities :

Conduct telephonic consultations with members for comprehensive medication reviews and adherence consults

Document care delivered and communicate essential information to primary care providers

Utilize prescription claims data to generate clinical recommendations and assess medication regimens

Required Qualifications :

Must be 18 years of age or older

Current and unrestricted Pharmacist license in the state of residence

1+ years of experience working remotely / independently in a cross-functional team environment

Solid knowledge of disease state management guidelines and MTM services

Intermediate proficiency with Microsoft Word, Outlook, and Excel
