Job Description

American Traveler is hiring an experienced Hospice RN for home-based care in LA County, requiring a CA RN license and at least one year of recent hospice experience.

Responsibilities

Home-based hospice care setting across LA County

8-hour day shifts, Monday through Friday (7 : 00–15 : 00)

Admit two new hospice patients daily with potential to visit up to six patients per day

Caseload primarily consists of geriatric patients

No weekend or on-call requirements

Patient ratio is 1 : 1, with a standard productivity of 30 points per day

Documentation completed using HomeCare HomeBase (HCHB)

Travel required within a service area that may occasionally exceed a 50-mile radius

Timely completion of patient admissions and hospice care tasks is expected

Documentation tools are provided and must be returned at end of contract

Mileage reimbursement provided at IRS standard rates

Free parking is available

Scrubs of any color may be worn

Travel pairs are accepted but each nurse is assigned to a separate territory and must have their own transportation

First-time travelers are welcome to apply

Former staff must be separated for at least six months and approved by HR to be eligible

RTO must be submitted with application

Requirements

Active and current CA RN license

BCLS certification required

Minimum one year of recent skilled hospice nursing experience within the last three years

Proficiency with HomeCare HomeBase (HCHB) preferred

Must have active driver's license and reliable personal transportation

Timely communication and documentation skills required
