Job Summary
Pharmacists are responsible and accountable for the provision of safe, effective, and prompt medication therapy. Through various assignments within the pharmacy department, they provide support of the medication-use systems as well as deliver optimal medication therapy to pediatric patients with medical complexities and a broad range of disease states. Pharmacists proficiently provide patient-centered care and integrated pharmacy services with physicians, nurse practitioners, nurses, and other hospital disciplines.
Schedule : one weekend shift a month and holiday rotation
Duties and Responsibilities :
Delivers patient-centered care and provides pharmaceutical services throughout the hospital
- Participate with medical and nursing staff regarding decisions about medication use for individual patients including selection of non-drug therapy, medication selection, dosages, routes and methods of administration; medication therapy monitoring; provision of medication-related information; and counseling to individual patients and families.
- Maintain a medication profile for each patient. Upon receiving a new medication order and prior to dispensing the medication review and screen profile for drug interactions, allergies, incompatibilities, and contraindications.
- Review and interpret pediatric medication orders. Calculate pediatric dosages. Prepare, evaluate, and verify sterile and non-sterile compounded medications. Monitor medication therapy. Promptly resolve any questions regarding orders or patient status with the prescriber.
- Review new or unusual therapy for safety; provide training for other health care team members. In collaboration with the Director of Pharmacy determine if Ranken Jordan is a proper setting for delivering certain therapies based on drug’s safety, cost, monitoring requirements, and based on the patient’s specific condition.
- Participate in the management of medical emergency.
- Perform and document interventions and consults.
Participate in pharmacy operations and medication dispensing
Manage controlled substance records, invoices, and reconciliation.Assist with the management of equipment, medication, and pharmacy supply inventory including preventative maintenance, ordering, stocking, and returns.Participate in quality improvement activities related to all aspects of drug therapy. Constantly review and evaluate medication system to advance patient safety, maximize therapeutic outcomes, and control costs.Follow standards for the selection, distribution, and safe and effective use of medications throughout the hospital.Oversee pharmacy operations and technician and student activities.Assist with developing student and staff experiences / competencies and fostering relationships for teaching and training opportunities.Assist with medication preparation of sterile and non-sterile compounds, prepackaging, and unit dosing of medicationsRequired to work one weekend shift per month and rotate through holiday schedule.Other duties as assigned.Qualifications :
Required : Graduate of an ACPE accredited college of pharmacy.Required : Currently licensed in good standing as a pharmacist in the state of Missouri.Required : Minimum 3 years pharmacist experience or 2 years Ranken Jordan Pharmacy experience.Preferred : Pediatric healthcare experience.Preferred : 1-3 years hospital experience.Physical Demands :
These include walking, stooping, kneeling, crouching, reaching, grasping, pulling, pushing, standing, carrying and lifting of light loads (up to 35 lb.) as necessary.
