About the RoleWe are partnering with a mission driven provider to hire a
Case Manager serving adults and families in a
shelter environment. You'll support intakes, stabilization planning, and rapid connections to housing and wrap around services.
What You'll Do- Conduct intakes/assessments; develop individualized housing stabilization plans.
- Connect clients to housing navigation, benefits, employment, and behavioral health services.
- Coordinate with landlords, outreach teams, and community partners; advocate for clients.
- Maintain timely, compliant documentation (e.g., HMIS/case management system).
- Provide crisis intervention and de escalation support as needed.
What You'll Bring- 1+ year in case management, shelter/interim housing, or homeless services.
- Knowledge of LA housing/CoC resources; HMIS experience preferred.
- Empathy, boundary setting, cultural humility, and strong time management.
- Bachelor's in Social Work, Psychology, or related field preferred.
