About the Role :
As a Social Worker in a Hospice Agency, you will play a crucial role in providing compassionate support to individuals and families facing end-of-life challenges. Your primary objective will be to ensure that patients receive holistic care that respects their dignity and preferences while navigating the complexities of hospice services. You will collaborate with healthcare teams to develop and implement care plans that address both emotional and practical needs. By employing crisis intervention techniques, you will help clients cope with grief and loss, facilitating a smoother transition during difficult times. Ultimately, your work will contribute to enhancing the quality of life for patients and their families during one of the most sensitive periods of their lives.
Minimum Qualifications :
- Master's degree in Social Work (MSW) from an accredited institution
- Current state licensure as a Social Worker
Preferred Qualifications :
Experience working in hospice or palliative care settingsCertification in hospice and palliative care social workResponsibilities :
Provide initial and ongoing psychosocial assessments for patients and familiesAssist in developing the plan of care as part of the interdisciplinary teamOffer emotional and grief support to patients, caregivers, and family membersFacilitate advance care planning and end-of-life decision-making conversationsConnect patients and families with community resources and support servicesMaintain timely, accurate, and compliant documentation in the EMRParticipate in IDG meetings, staff in-services, and case conferencesProvide crisis support and brief counseling interventions as neededParticipate in on-call rotation and make home visits as necessaryConduct assessments to determine the social, emotional, and financial needs of patients and their familiesDevelop and implement individualized care plans in collaboration with healthcare professionalsProvide counseling and support to patients and families, helping them navigate the emotional aspects of end-of-life careFacilitate communication between patients, families, and the healthcare team to ensure that care aligns with the patient's wishesEducate families about available resources and services, including financial assistance and community supportWhat We Offer :
