About The Position

Job Title : Home Health Physical Therapist Profession : Therapy / Rehabilitation Specialty : Physical Therapy (Home Health) Duration : 13 Weeks Shift : Day Hours per Shift : 8 Experience : Minimum of 1 year in Home Health experience required License : Physical Therapy license required in applicable state Certifications : BLS required; additional specialized certifications preferred Must-Have : Epic charting experience preferred; ability to assess adaptive and assistive devices; knowledge of vital sign monitoring; familiarity with cardiopulmonary patient management Description : Position available for a Home Health Physical Therapist. The role involves providing therapy to patients recovering from illnesses, injuries, or surgeries directly in their homes. Working hours are Monday through Friday from 8 : 00 AM to 4 : 30 PM. Knowledge and experience in assessing and utilizing adaptive devices and identifying environmental barriers is essential. Candidates should be capable of monitoring vital signs and providing gait training for orthopedic conditions. Experience with various patient populations, including geriatrics and those with cognitive disorders, is necessary. Candidates will be responsible for creating and managing plans of care based on individual patient assessments. The position requires proficiency in documentation and billing processes for Medicare, Medicaid, and private pay. Strong time management skills are also necessary to effectively schedule visits and manage patient care. Travel may be required within a specific region. Mileage reimbursement is provided for travel-related expenses. Specialized skills in cardiopulmonary care and managing chronic conditions are preferred. Candidates should have experience with long-term and short-term goal setting for patient rehabilitation. Candidates will be expected to commit to a holiday shift during their assignment period.
