• This is a virtual position based out of our Command Center in Charlestown, MAClinical outcomes are equal to or better than inpatient hospitalization.Work in a team environment (Physicians, NPs, RNs) in a virtual Command CenterMedically Home significantly reduces total medicine expense from avoidable inpatient visits, post-acute care and readmissions.Improved patient experience.State of the art equipment at Command Center and patients home.Patients have home infusion, meals / nutrient consults, pharmacy / medical mgmt., mobile imaging, laboratory and DME / oxygen.Integrated care with primary care providers, specialists & ancillary departments.Burlington, Massachusetts, is a thriving town with a population of approximately 27,000, known for its strong sense of community and excellent quality of life.
