Physical Therapist (PT) Skilled Nursing

About the Role :

As a Full-Time Physical Therapist , you'll play a vital role in assessing, treating, and guiding residents through personalized rehabilitation plans. You'll be part of an interdisciplinary team committed to delivering exceptional care within a skilled nursing and long-term care setting.

Key Responsibilities :

Perform comprehensive evaluations and develop individualized treatment plans.

Use evidence-based physical therapy techniques to improve residents mobility, strength, and function.

Provide pain management through manual therapy, therapeutic exercise, and adaptive equipment.

Educate residents and families on therapy objectives, outcomes, and recovery expectations.

Adjust care plans based on clinical assessments and patient progress.

Supervise and mentor physical therapy assistants when applicable.

Maintain accurate, timely documentation in compliance with state and facility regulations.

Collaborate with nursing staff, occupational therapists, and other care providers to ensure holistic, patient-centered treatment.

Qualifications :

Degree from an accredited Physical Therapy program .

Current New York State Physical Therapist (PT) license in good standing.

Experience in a skilled nursing facility (SNF) or long-term care setting preferred.

Strong interpersonal skills with a compassionate and patient-centered approach.

Ability to work effectively within a multidisciplinary healthcare team.
