Job description

Job Description

Seeking a dedicated Speech Therapist (ST) for home health on a PRN basis, offering flexibility and rewarding 1 : 1 patient care.

Responsibilities include :

  • Providing skilled speech therapy in patients' homes
  • Creating and implementing individualized care plans for communication and swallowing goals
  • Accurate documentation in Axxess EMR
  • Collaborating with the care team and educating patients and families

Requirements :

  • Active Illinois Speech-Language Pathologist license
  • At least 1 year of clinical experience, home health preferred
  • Reliable transportation and willingness to travel 100%
  • Strong communication, time management, and documentation skills
  • CPR certification
