Position Summary : We are seeking a dedicated and skilled Licensed Practical Nurse (LPN) to join our Medical Unit 4E team. The ideal candidate will demonstrate exceptional patient care skills, proficiency in telemetry, and a strong understanding of Epic systems. This role involves distributing medications, assisting with care, and collaborating with a multidisciplinary team to ensure optimal patient outcomes.
Key Responsibilities :
Assess and evaluate patients' physical, psychosocial, environmental, and educational needs, collaborating with the team to achieve desired outcomes.Support the development and implementation of care plans, including discharge planning, by applying critical thinking and clinical expertise.Facilitate patient and family education as outlined by the Registered Nurse (RN), tailoring learning opportunities to individual needs.Adapt proactively to changes in the healthcare environment by embracing new technology and processes.Communicate effectively with RNs and multidisciplinary caregivers regarding changes in patient conditions.Attend departmental meetings and educational opportunities with a required participation of 80%, including in-person and webinar attendance.Maintain confidentiality, adhering to HIPAA regulations and safety protocols.Document patient information accurately in the electronic and / or paper medical record systems.Delegate and receive tasks appropriately, based on patient needs and scope of practice.Participate in process improvement teams and support initiatives enhancing patient satisfaction and clinical outcomes.Take responsibility for personal and professional development, setting yearly performance goals and advancing clinical expertise.Serve as a steward of hospital resources, meeting unit productivity expectations and supporting staffing needs across departments when required.Requirements :
Education / License : Active and valid BLS and LPN license.Experience : Minimum of 1 year of relevant clinical experience.Skills : Proficiency in Epic and telemetry, with a commitment to providing competent and compassionate patient care.
