  • Average Daily Volume / Census : Outpatient clinic-based caseload
  • Admissions & Rounding : Not required; hospitalist team available

  • Case Mix / Clinical Focus Areas : Primary care, preventive medicine, chronic disease management, wellness FACILITY & TEAM DETAILS
  • Beds in Department / Unit : Outpatient-based practice, affiliated with 4-hospital health system
  • EMR System : Epic
  • Specialty Backup : Multispecialty support throughout the system
  • Support Staff : Nurse practitioner and ancillary support available
  • Leadership & Reporting Structure : Physician employed through Community Care Network, providing services to 219 Health Network Inc. (FQHC-Look Alike)

Requirements : BC / BE in Family Medicine or Internal Medicine

  • DEA & Certifications : Active DEA; BLS / ACLS required

  • Deliver comprehensive primary care services in an outpatient setting
  • Provide preventive, acute, and chronic disease management
  • Collaborate with nurse practitioners, specialists, and hospitalist teams as needed
  • Participate in a 1 : 4 outpatient call rotation
