Job Description
The Social Services Coordinator supports the delivery of high-quality psychosocial services to residents and their families within the center. This role is actively involved in assessments, care planning, admissions and discharges, and resident rights advocacy. The Social Services Coordinator collaborates with the interdisciplinary team to promote resident-centered care, maintain regulatory compliance, and foster a supportive community environment.
Qualifications :
- Minimum of 2 years of social work experience in healthcare settings, with at least 1 year being in long-term care of skilled nursing, required.
- High School Diploma or GED, required.
- Bachelor's degree in Social Work or related field, preferred.
- Experience in a leadership role, preferred.
- Experience with MDS assessments and Medicare / Medicaid regulations, preferred.
- Comprehensive knowledge of federal and state regulations
- Strong clinical assessment and intervention skills required.
- Excellent interpersonal and communication skills required.
- Demonstrated leadership capabilities, preferred.
