Program Manager (49181)

Position Summary :

Person in this position will be responsible for providing administrative programmatic leadership and oversight of the program of the Department of Social Services (DSS) Homeless Disability Advocacy Program (HDAP). Responsible for coordination of client care by collaborating with multidisciplinary professionals to provide and facilitate services. This position will monitor data for contract compliance, submit performance reports, and will provide supervision through direct observation and follow-up training. Essential Job Functions include those listed below.

Essential Job Functions :

• Oversight of day-to day operations of the Housing Disability Advocacy Program (HDAP) program.
• Oversight and Management of operating budget and program fiscal expenditures.
• Oversight of the intake and assessment of incoming participants to determine eligibility and appropriate services.
• Knowledge of the SSI / SSDI Outreach, Access, and Recovery (SOAR) model.
• Responsible for reviewing SOAR Medical Summary Reports (MSRs).
• Hire and arrange for staff development, education, and training activities.
• Function as a liaison to the Director of Housing Services external groups and agencies in promoting inter-organizational collaboration including attending Community meetings as required.
• Monitor compliance with all required standards, regulations, state, and federal guidelines.
• Heavy documentation; Homeless Management Information System and WestCares Clinical Data System.
• Responsible for collection and submission of weekly, monthly, and quarterly reports.
• Develop, implement, and monitor procedures to meet agency policies and contract management including the preparation of comprehensive reports for funding or agency sources.
• Embrace and embody the mission, vision, guiding principles, clinical vision, and goals of WestCare Foundation; and
• Perform any other duties as assigned.
