- Trinity Health Grand Rapids provides care for patients in eight local nursing facilities and supports the Trinity Health at Home Thrive Program, an Advanced Care Coordination Program.

- Open to new graduates who have completed a Fellowship in Geriatrics
- Open to experienced candidates in Internal Medicine or Family Medicine
- If candidate is TRULY Board Eligible, must become Board Certified within 2 years of becoming eligible
- Not accepting candidates who have lapsed Board Certification and need to obtain again
