Job Description :

• Trinity Health Grand Rapids provides care for patients in eight local nursing facilities and supports the Trinity Health at Home Thrive Program, an Advanced Care Coordination Program.
• Opportunity to teach Residents, Medical Students, and Fellows
• Practice leadership and team that supports work / life balance
• Autonomy and flexibility in schedule
• Visit length adequate to meet complex patient population
• Call shared among Providers
• Open to new graduates who have completed a Fellowship in Geriatrics
• Open to experienced candidates in Internal Medicine or Family Medicine
• If candidate is TRULY Board Eligible, must become Board Certified within 2 years of becoming eligible
• Not accepting candidates who have lapsed Board Certification and need to obtain again
• Not open to commuters and / or those looking for secondary employment
• Not accepting Locums candidates
• Not accepting visa candidates
