Delaware Licensed Resource Coordinator

A company is looking for a Resource Coordinator II.

Key Responsibilities

Assist Behavioral Health members post-emergency department visits with appointment setting and service linkages

Coordinate discharge plans and address social determinants of health through community resource connections

Assess members' needs via telephone / virtual interactions and schedule follow-ups as necessary

Required Qualifications and Education

Associate's degree in healthcare, human services, or a related field, or equivalent work experience

Active Licensed Practical Nurse (LPN) can substitute for the required education

3 to 5 years of experience in maternity, human services, and community programs

Knowledge of medical terminology and understanding of Medicaid requirements

Familiarity with managed care policies and procedures
