Le travailleur social procède à l’évaluation des besoins psychosociaux de la personne et de son environnement. Il soutient et rétablit le fonctionnement social de l’individu. Il met en place un plan d’intervention et oriente l’usager vers les bonnes ressources. Finalement, il s’assure de la continuité et de la bonne cohésion de tous les services, dans le but de favoriser le développement optimal de l’usager.
Plusieurs assignations sont disponibles auprès de différentes clientèles :
•DITSA (Déficience intellectuelle et trouble du spectre de l’autisme);
•GMF;
•Réadaptation physique;
•Santé mentale adulte et dépendances;
•Soutien à domicile, liaison gériatrique et maladies chroniques.

Exigences
Ce que tu dois avoir!
•BAC en travail social ou en service social;
•Être membre de l’Ordre des travailleurs sociaux et des thérapeutes conjugaux;
•Aimer travailler en équipe multidisciplinaire;
•Avoir une facilité à communiquer et bonne capacité d’écoute;
•S’adapter à différents types de milieux et de clientèles.
