Job Description

We are currently seeking a skilled and compassionate Primary Care Nurse Practitioner to join our team in Defiance, Ohio. This is a full-time outpatient role focused on providing primary care services to adult and geriatric patients. The ideal candidate will bring strong clinical expertise in adult-gerontology and a passion for delivering patient-centered care.

Key Responsibilities :

Provide primary and preventative care for long term care patients residing in skilled nursing facilities.

Manage Residents with acute change in condition.

Advanced care planning and chronic care management.

Prescribe appropriate medications, including controlled substances in accordance with OH Law.

Order, review and / or follow-up with treatments and diagnostic testing.

Communicate with the nursing home staff on a regular basis and as needed to discuss changes in condition.and / or the care plan.

Collaborate with a primary care physician.

Required Qualifications :

Master’s Degree in Nursing with a specialization in Adult-Gerontology Nurse Practitioner (AGNP) – Required

Current Nurse Practitioner License in the State of Ohio – Required

Board Certification as an Adult-Gerontology Nurse Practitioner (AGNP) – Required

DEA License Required

Minimum 2 years of experience in primary care, internal medicine, or geriatric care preferred

Strong clinical judgment, communication, and interpersonal skills

Proficient with electronic medical records (EMR) systems
