Job descriptionDeliver services : Skill-building, behavioral interventions, psychoeducation, and care coordination that support recovery and daily functioning. Own the plan : Co-create, implement, and review Connect resources : Link to housing, education, employment, healthcare; support navigation and follow-through. Use evidence-based approaches : e.g., behavior modeling / modification, Motivational Interviewing. Guide paraprofessionals : Provide direction so interventions are consistent, effective, and aligned to the plan. Assess & document : Complete initial / ongoing assessments; enter accurate, timely notes in the EHR (Notenetic preferred)generally within 24 hours of service and per agency / Medicaid standards. Team & partners : Join weekly huddles; liaise with DSS / MCOs, primary care, housing providers, and other partners to keep care seamless. Crisis response : Respond promptly; de-escalate, stabilize, and follow agency protocols. Like field-based work and meeting people where they are (home, community, clinic). Balance heart and rigorclient-centered care with organized, compliant documentation. Enjoy coaching paraprofessionals and improving workflows. Communicate clearly, manage time well, and adapt quickly when needs change. Meets NC QP criteria : Masters in Human Services and 1+ year with the target population. Current / eligible First Aid / CPR, Bloodborne Pathogens, and NCI / CPI Part A (or willingness to obtain upon hire). Valid drivers license and reliable transportation for local travel. Proficiency with EHRs and productivity tools (Notenetic preferred; Microsoft 365 / Google Workspace). Ability to maintain professional boundaries and confidentiality (HIPAA). Community-based role with regular local travel across the service area. Flexibility for evening / weekend coverage as client needs arise.
