STANDBY RATE : $150 for DAY / NIGHT

ON-CALL PER VISIT : $75 / VISIT

On-Call Hours : 5 : 30pm - 8 : 30am

Job Summary : American Hospice and Home Health Services is seeking a dedicated Part-Time On-Call LVN to provide compassionate care to patients in their homes.

Our mission in home health care is to assist patients in achieving the highest level of independence in their daily activities.

Essential Job Functions / Responsibilities :

• Deliver nursing care and supportive services in alignment with the physician’s treatment plan.
• Conduct accurate assessments and correlate findings to establish an effective plan of care.
• Monitor patient progress, document observations, and implement physician and nursing treatment plans.
• Maintain all documentation per policy and procedure.
• Communicate effectively via phone, email, and written communication with healthcare professionals.
• Work collaboratively with team members to provide optimal care.
• Manage visit scheduling and documentation time efficiently.
• Learn and utilize an electronic medical records system.
