Job description
Shift : Day
Working Hours : Monday - Friday, 8am - 5 pm
Summary :
Acts as primary therapist and case manager / case coordinator for a group of patients. Coordinates, directs, formulates, and participates in all aspects of the treatment experience with the rest of the interdisciplinary team. Documents treatment progress and ensures continuity of care through discharge, including discharge planning.
Other information :
Masters Degree in Social Work or related mental health field, licensure as a "mental health professional" and a minimum of two (2) years experience.
This job will be authorized 80.00 hours bi-weekly.
