Job description

We are working with an established facility in Enfield, CT that is currently in search of a nurse practitioner or physician assistant to bring permanently onto their team.

FNP's and PA's need to be board certified and hold an active CT state license.

Schedule :

Full time hours, Monday - Friday, typically 8-5PM

Rotating Saturday on call hours maybe once a month (4 hours)

Call is 1 day every 50-60 days and we have a 24 hr call center with triage nurses. Average on call volume is between 0-5 calls.

Requirements :

Prior Primary Care / Family Practice experience required

More than 18 months required

Epic experience required

Active STATE license needed

50% care coordination (phone calls from their patients, admin stuff, follow ups), 50% in office outpatient care
