Our Mobile Primary Care team delivers care directly into homes, assisted living, and skilled nursing facilities - serving complex senior populations through a value-based, panel-driven model.

This role is designed for clinicians who want more than a job. It is structured for providers who can successfully build and manage a senior panel - and grow into future clinical leadership roles.

You are a Physician Assistant who :

• Has experience in home-based care, SNF, ALF, or senior living communities
• Understands panel management and long-term patient relationships
• Practices confidently and independently in the field
