Care Coordinator

A company is looking for a Care Coordinator (Remote).

Key Responsibilities

Act as the telehealth concierge and first point of contact for patients, conducting intake appointments and answering questions

Match patients with appropriate physicians or care pathways and manage their telehealth appointments

Collaborate with clinical teams and participate in process improvement initiatives to enhance patient experience

Required Qualifications

1-3+ years of experience in direct patient care, digital health, or care coordination

Proficiency with CRM platforms, scheduling systems, and telehealth software

Experience achieving measurable KPIs related to patient engagement and satisfaction

Ability to manage a high volume of patient interactions effectively

Familiarity with insurance and health plan benefit structures is preferred
