Seven Healthcare is pleased to offer a travel nursing opportunity for a Registered Nurse - Case Manager in Fairfield, CA. This contract provides a competitive weekly rate of $3,700, based on five 8-hour day shifts.

Key Responsibilities of the Travel Case Manager Nurse :

  • Coordinate and oversee the patient's continuum of care across various healthcare settings
  • Conduct comprehensive assessments and develop individualized care plans
  • Collaborate with physicians, nurses, social workers, and other care team members to ensure seamless care transitions
  • Monitor patient progress and modify care plans as needed to achieve clinical and financial goals
  • Provide education and support to patients and families regarding diagnoses, treatment options, and community resources
  • Facilitate discharge planning, including referrals to home health, rehabilitation, or long-term care facilities
  • Maintain compliance with hospital policies, regulatory standards, and documentation requirements
  • Participate in performance improvement initiatives and case reviews
  • Requirements of the Travel Case Manager Nurse :

  • Active California Registered Nurse (RN) license
  • Current BLS certification
  • Certified Case Manager (CCM) or Accredited Case Manager (ACM)
  • Minimum of 2 years acute clinical nursing experience
  • Some weekended coverage needed (Mostly on Saturdays)
